Christian Education and Youth Group Forms

Link for Registration Form for 2011-2012 : Fill this form out to register for our church school and/or youth group programs in the 2011-2012 school year.

Link for Medical Form/Permission Slip  : Fill this form out for children who are registered in our church school or youth group programs, and who could be attending programs at which a parent is not present. We suggest that all parents submit this with the registration form.

Link for Volunteer Guidelines and Application Form : All volunteers in our church school and youth group programs are required to fill out this form and submit it with a copy of their driver's license. An authorization form for us to conduct a criminal background check also needs to be filled out. 

Link for Drivers Application : All drivers for church events and programs should fill out this form if they will be transporting persons outside of their own family.
